Starting Pay Rate Range: $49.78 – $56.50, commensurate with experience

This position may be filled as either full-time (1.0 FTE) or part-time (0.5 FTE), based on clinical needs and the qualifications and availability of the selected candidate.

Summary
The Clinical Pharmacist is a full-time or part-time ambulatory care position integrated in a primary care practice which is responsible for the execution of medication therapy management (MTM), disease state management through collaborative practice agreements (CPA) and provision of immunization services. Position involves precepting and supervision of pharmacy interns, residents, and/or other employees.
Principal Functional Responsibilities
Pharmacy Services: Deliver telehealth and in-person pharmacy services, including but not limited to:
  • Comprehensive Medication Reviews (CMR)
  • Targeted Medication Reviews (TMR)
  • Medication Reconciliations
  • Disease management through CPA
  • Substance use disorder treatment
  • Travel health consults

Clinic Services: Manage and expand immunization clinic services to both onsite and offsite clinics.
Miscellaneous Duties: Perform other duties as assigned.
Qualifications
  • Doctor of Pharmacy (PharmD) degree from an ACPE accredited institution
  • Registered pharmacist in the State of Ohio with an active status
  • Active CPR license (if applicable)
  • Immunization training (if applicable)
  • Proficient with Microsoft Office Suite
Preferred Qualifications
  • Provision of MTM services, medication reconciliations, and chronic disease management
  • Completion of Immunization Certificate Training program
  • Experience in developing immunization services, including securing new vaccine clinics
  • Experience in precepting and supervising pharmacy interns, and/or pharmacy residents
Physical Requirements
Must be able to utilize a phone, computer, and other office equipment. May be required to work between two different University campuses located less than 10 miles apart.
