Position Summary Information
Summary
The Clinical Pharmacist is a part-time position which is responsible for the execution of medication therapy management (MTM) and disease state management through collaborative practice agreements (CPA). The Clinical Pharmacist will have the opportunity to be involved in additional services, such as seasonal immunization clinics.
Principal Functional Responsibilities
Pharmacy Services: Deliver telehealth pharmacy services, including but not limited to:
a. Comprehensive Medication Reviews (CMR)
b. Targeted Medication Reviews (TMR)
c. Medication Reconciliations
d. Travel Health Consults
Miscellaneous Duties: Perform other duties as assigned.
Qualifications
  • Doctor of Pharmacy (PharmD) degree from an ACPE accredited institution
  • Registered pharmacist in the State of Ohio with an active status or eligible for licensure in the State of Ohio
  • Active CPR license (if applicable)
  • Immunization training (if applicable)
  • Proficient with Microsoft Office Suite
Preferred Qualifications
  • Provision of MTM services, medication reconciliations, and chronic disease management
Physical Requirements
Must be able to utilize a phone, computer, and other office equipment.
