Position Summary Information
Summary
The Clinical Pharmacist is a full-time ambulatory care position which is responsible for the execution of medication therapy management (MTM), disease state management through collaborative practice agreements (CPA) and immunization services. Position involves supervision of paid and un-paid pharmacy interns, pharmacy residents, and/or other employees.

· Days: Monday through Saturday
· Hours: Variable (8 AM – 7 PM)

Pay Grade: 14
Starting Salary Range: $100,518-$120,622, commensurate with experience
Location: Rootstown, OH

Principal Functional Responsibilities
1. Deliver telehealth and in-person pharmacy services, including but not limited to:
a. Comprehensive Medication Reviews (CMR)
b. Targeted Medication Reviews (TMR)
c. Medication Reconciliations
d. Disease management through CPA
e. Substance use disorder treatment
f. Travel health consults

2. Manage and expand immunization clinic services.
a. Onsite clinics
b. Offsite clinics

3. Other duties as assigned.
Qualifications
Requirements:
  • Doctor of Pharmacy (PharmD) degree from an ACPE accredited institution
  • Registered pharmacist in the State of Ohio with an active status
  • Must have an active CPR license
Experience:
  • Provision of MTM services, medication reconciliations, and chronic disease management highly preferred
  • Experience in administering immunizations
  • Willingness to precept pharmacy students and/or interns

Physical Requirements
Must be able to utilize a phone, computer, and other office equipment. While performing the duties of the job, the employee is frequently required to sit; talk; or hear and occasionally stand, walk, reach, stoop, or kneel. Must be able to occasionally lift up to 25 pounds.
